How it Works

What is Care Management?

Care management is a service often offered telephonically by provider offices and/or insurance companies in order to improve the health of the client. Care managers help the client manage their medical condition(s), coordinate care, and navigate the complexities of the health system. They do this by gathering information about your social and medical history, providing education about your medical condition(s), and reinforcing your provider(s) instructions regarding medication and lifestyle changes.

Together with the care manager, you will come up with a plan, called a care plan, to move you forward on your health journey and discuss any barriers you might have. The care manager will work with you to find ways around the barriers as you move towards improving your health. This is your journey and you are an active participant in the process.

Sophie has her Bacherlor's degree in psychology and has been a registered nurse since 2015. After she developed her own complex medical condition, she found the perfect job in care management. She was able to use her education as well as her personal experience to help people navigate the health system and understand their medical condition(s).

She will meet you where you are without judgement. You'll talk about your goals and what is holding you back. Together, you will come up with a plan to reach those goals. She understands that not every day is going to be perfect, but by reflecting on what works and what doesn't, the good days can start to outnumber the bad.

Whether you need to make lifestyle changes to improve your health, education about a medical condition, have your own complex health condition you are trying to navigate, or don't know what the next step should be on your health journey, Sophie is here to help. She's been where you are and she understands.

Scope of Service Disclaimer

Sophie is a Registered Nurse licensed in Maryland and holds a multistate license under the Nurse Licensure Compact (NLC). In states that recognize the NLC, she may offer independent care management services within the full scope of her nursing license, including clinical coordination, health advocacy, and personalized care planning.

In all other U.S. states and internationally, she can offer non-clinical health coaching services focused on lifestyle optimization, behavior change support, and wellness education. These services do not constitute medical or nursing care and are not a substitute for diagnosis, treatment, or clinical decision-making.

Regardless of location, she does not diagnose conditions, prescribe medications, or act as a treating provider. Clients are encouraged to consult their local healthcare team for medical concerns or treatment decisions.
